MAKING YOUR PLAN WORK FOR YOU
What is NDIS support coordination?
Support coordination is NDIS-funded help to understand and use your plan. A support coordinator connects you to services, sets up providers, manages your plan supports and builds your skills to handle the NDIS yourself over time. It sits in the Capacity Building budget and is delivered in three levels — Support Connection, Support Coordination and Specialist Support Coordination — depending on how complex your needs are.
At Essence Community Care, our Melbourne support coordinators start by listening — to you, your family and the people who know you best. From there we help you find and set up the supports in your plan, resolve problems when they come up, and prepare you for your next plan review, so your funding is actually used the way it was meant to be.
Support coordination is different from plan management. A plan manager pays your invoices and tracks your budget; a support coordinator helps you decide who to use, builds your service network and grows your capacity to self-navigate. Many participants have both. We explain the difference plainly and help you work out what your plan needs.
Whether you are new to the NDIS, moving out of hospital, or your situation has changed since your last plan, we take the time to understand what a good outcome looks like for you — then coordinate the supports to get there.
- Connecting you to disability, health and community services across Melbourne
- Setting up service agreements and bookings with your chosen providers
- Coordinating complex or crisis situations calmly and quickly
- Building your skills so you rely on us less over time, not more

Support coordination across Melbourne
Our office is in Coburg, in Melbourne’s inner north, and our coordinators support NDIS participants right across the metro area. Because we’re based locally, we know the disability, health and community services in Melbourne’s north especially well — which means we can connect you faster to supports that actually fit.
We work with participants right across greater Melbourne, including Coburg, Brunswick, Preston, Northcote, Fitzroy, Fawkner and Reservoir in the inner north; the Melbourne CBD, Carlton, Richmond and Footscray closer in; and Broadmeadows, Craigieburn, Sunshine, Dandenong and Box Hill further out. We can meet in person where it suits you, or work by phone and video if that’s easier.
Not sure whether we cover your suburb? Just ask — we’ll tell you straight away.

Common questions
Support coordination — frequently asked questions
A support coordinator helps you understand and use your NDIS plan. In Melbourne, that means connecting you to local disability, health and community services, setting up your providers and bookings, resolving problems, and building your skills to manage the NDIS yourself over time. At Essence Community Care, our Coburg-based coordinators do this across the whole Melbourne metro area.
If support coordination is funded in your NDIS plan, you can choose any provider you like — you don't have to use the one the NDIA suggests. Look for a registered provider that services your area, ask how they'll help you reach your goals, and check they're responsive. You can contact Essence Community Care on 0413 327 992 to get started.
Yes. Support coordination is funded under the Capacity Building budget in your NDIS plan. It is separate from your core supports, so using a coordinator does not reduce the funding you have for day-to-day support workers. If it isn't in your current plan, you can request it at your next review.
Yes. Choice and control is a core NDIS principle. If your plan includes support coordination funding, you decide which provider delivers it and you can change providers if it isn't working. You are never locked in to one coordinator.
A plan manager handles the money — paying your invoices and tracking your budget. A support coordinator helps you decide who to use, sets up your services and builds your capacity to navigate the NDIS. They're funded from different budgets, and many participants have both.
You don't pay out of pocket — support coordination is paid from the funding in your NDIS plan at the NDIS price limits. The exact hourly rate depends on the level of coordination in your plan (Support Connection, Support Coordination or Specialist Support Coordination). We'll always work within your funded budget and be transparent about hours used.
It depends on your situation. If your plan is straightforward and the NDIS portal feels manageable, you may be fine self-navigating. If your needs are complex, you're new to the NDIS, or you simply don't have time to chase providers, a support coordinator saves you effort and helps you avoid underspending your plan.
Yes. Essence Community Care is a registered NDIS provider and meets the NDIS Quality and Safeguards Commission standards. Our support coordination is delivered from our Coburg office across Melbourne.
